Complete registration here for Ephrata Jr Tigers 2025 Tackle football season. There will be a 3rd/4th grade team as well as a 5th grade team and a 6th grade team. Please make sure to upload the following documents per league standards.
Player Enrollment Photo: Must be shoulders up clear photo of the player only. No hats, hoods, bandanas, sunglasses, helmets.
Player Birth Certificate: Please upload players birth certificate clearly showing player name, DOB & Parent Names. Alternative: Valid (non-expired) Passport. Not Accepted: Hospital birth
certificates.
Player Grade verification: Please upload Players Skyward Online Student Profile showing Student name, Date of Birth, Address, Grade & Graduation year. All items must be showing
Address Verification: Please upload any piece of mail showing Registered Parent Name, Date within previous 30 days, & Address on account. Accepted docs: Mortgage Statement dated within past 30 days, Current, unexpired Rental/Lease agreement, piece of mail dated within the past 30 days. Not accepted: PO Boxes, Student Online Profile and mail addressed
to “current resident”.
