CODE OF CONDUCT

I, A PARENT OR GUARDIAN of the above named player agree that the Players and I will abide by the rules of the EPYSA, CPYSL, BSYSA and any affiliated organizations and/or sponsors.  Recognizing the possibility of physical injury associated with soccer and in consideration 

 OF THE ABOVE NAMED PLAYER, A MINOR, AGREE THAT THE PLAYER AND I WILL ABIDE BY THE RULES OF THE EPYSA, CPYSL, BSYSA AND ANY OF THEIR AFFILIATED ORGANIZATIONS AND/OR SPONSORS. RECOGNIZING THE POSSIBILITY OF PHYSICAL INJURY ASSOCIATED WITH SOCCER AND IN CONSIDERATION FOR THE EPYSA, CPYSL, BSYSA ACTIVITIES, (THE PROGRAMS), I HEREBY RELEASE, THEIR EMPLOYEES, VOLUNTEERS INDEMNIFY THE EPYSA, CPYSL, BSYSA, THEIR AFFILIATED ORGANIZATIONS AND SPONSORS, THEIR EMPLOYEES, VOLUNTEERS AND ASSOCIATED PERSONNEL INCLUDING THE OWNERS OF THE FIELDS AND FACILITIES UTILIZED FOR THE PROGRAMS, AND/OR BEING TRANSPORTED TO THE GAME, WHICH TRANSPORTATION HEREBY AUTHORIZE. ADDITIONALLY, I GIVE PERMISSION FOR AND HEREBY AUTHORIZE ALL MEDICAL ATTENTION NEEDED BY THE PLAYER IN THE EVENT OF AN ACCIDENT, INJURY, SICKNESS, ETC. WHILE PARTICIPATING IN THE PROGRAM AND THAT THE SAME BE GIVEN UNDER THE DIRECTION OF THE PLAYER’S COACH UNTIL SUCH TIME AS I MAY BE CONTACTED. I ALSO HEREBY ASSUME THE RESPONSIBILITY OF PAYING FOR SUCH TREATMENT.



The main goal of our organization is to maximize the opportunity for each registered player to enjoy and learn about the sport of soccer.  While participating at a competitive level, we encourage a daily display of sportsmanship, acquisition of skills, and building of the team idea.


Player Agreement:  I agree to play to the best of my ability during the upcoming season or until I am given my release in writing by BSYSA.  


And I will:

1.  Refrain from the use of any profanity when participating in BSYSA activities.

2.  Be modest when successful and be gracious in defeat.

3.  Be courteous to opposing teams and treat all players and coaches with respect.

4.  Demonstrate good sportsmanship before, during and after games.

5.  Show respect for authority to the officials of the game and of the league.

6.  Respect the game, play fairly, and follow its rules and regulations.

7.  Respect the privilege of the use of public facilities.

8.  Not be under the influence of any drug or alcohol.


Parents Pledge:  I recognize that parents are the most important role models for their children and that amateur athletics help to develop a sense of teamwork, self-worth and sportsmanship.  I encourage my child to play by the rules and respect the rights of others.  I understand it is important to enforce rules of play and set conduct standards as necessary components in athletics and life.  I will at all times encourage my child to play by the rules, respect the game official's decision and not criticize a game official's ruling during or after an athletic event.


And I will:

1.  Place the well-being of my child before a personal desire to win.

2.  Encourage good sportsmanship by demonstrating positive support for all players, coaches, fans, and officials at games, practices and other sporting events.

3.  Encourage my child to play by the rules and respect the rights of other players, coaches, fans and officials.

4.  Supporting a "Team" environment by having my child at scheduled practices and games.

5.  Not be under the influence of drugs or alcohol
