Player Petition Form

The Board of Directors review all petitions submitted. Petitions are approved by a majority vote of the board and also must meet any specific requirements pending the type of petition submitted. You must provide all necessary, relevant information on this form for your request to be considered. 

Petitions are request only; they're not guaranteed to be granted.
